Benefits
The following
summary identifies the key elements of your benefit package. This document
is intended only as a summary of those benefits. The full information
must be obtained from the certificates of insurance, and other appropriate
documents.
All eligible employees will receive at no cost the following benefits:
- Long term disability benefits
- Short term disability benefits
- $50,000 term life insurance for employees under 65; upon the age
of 65, $32,500; and at age 70, $25,000
- Accidental death and dismemberment insurance equaling up to the
term life coverage for employees (does not include the voluntary term
life insurance that the employee may elect to purchase)
- Emergency Travel Assistance provides emergency medical and personal
assistance when you or any eligible member of your family becomes ill or
injured while traveling just 100 miles or more away from home for business or
pleasure, domestically or abroad
- Annual vision screening benefit for employees and their families
with a co-pay of $10 per exam.
The following optional benefits are available for eligible employees
and their families at low group rates:
- Additional term life insurance for employees, their spouses and
children
- Comprehensive major medical insurance that pays 90% in-network or
70% out-of network of covered charges after your deductible has been
met. Our deductibles and out-of-pocket expenses are non-combined.
Meaning if you use both in-network and out-of-network providers you
will satisfy two separate deductibles and out-of-pocket expenses.
If you use only in-network or only out-of-network providers you will
satisfy one deductible and one out-of-pocket expense. For in-network
providers, your annual deductible is $250 for single/$500 for family
with an annual maximum out-of-pocket expense of $750 for single/$1500
for family. For out-of-network providers, your annual deductible is
$1000 for single/$2000 for family with an annual maximum out-of-pocket
expense of $2500 for single/$5000 for family.
- Employee dental plan designed to promote regular professional care.
Pays 100% for checkups and cleanings. Pays 80% for basic care after
a $25 deductible per person, and pays 50% per year for major care
after a $25 deductible per person each calendar year. The annual maximum
dental benefit is $1,500 per individual.
- Orthodontic (braces) benefits for your covered dependent children
under the age of 19. Cost of braces reimbursed at 50% up to a lifetime
maximum of $1000 per child. You and your dependents must be covered
under our dental plan for 24 months before orthodontic benefits are
available.
- Prescription drug benefits ($10 generic, $20 name brand copay).
- TRICARE/CHAMPUS Supplement Insurance which covers 100% of your TRICARE/CHAMPUS
deductible, the 25% cost share for outpatient and hospital services
and 100% of eligible expenses over the TRICARE/CHAMPUS allowable amount.
- Additional Vision Coverage may be purchased for frames and lenses.
When you purchase this benefit you may receive new lenses annually
for just $25 and new frames every two years for $25.
401(k) Plan with employer matching. Employer matches 50% of employee
contribution to a maximum of 3% (50% of employee's 6%).
Company paid credit cards for eligible travelers.
Educational reimbursement for 100% of educational expenses (tuition,
books, and lab fees for the grade of C or better) to a maximum reimbursement
of $5,250 per year through calendar year 2010. This applies to both
undergraduate- and graduate-level courses.
Salary differential is paid for up to five days of jury duty.
Salary differential is paid for up to ten days active duty military
training annually.
Ten days vacation per year. Fifteen days for employees with over five years
of service. (Amounts may vary for SCA employees.)
Five days paid sick leave per year.
Ten paid holidays per year.
